Leishmaniasis is a poorly investigated disease mainly affecting people in developing countries. Drug screening by the isolation of natural products seems to be an attractive approach which can result in the efficient elucidation of new lead compounds. This is a valuable option to standard screening of large compound libraries. Although a significant number of antileishmanial compounds have been investigated, the number of mechanistic studies is rather small. The actual target sites are unknown in most cases. It is important to highlight that the late discovery process, such as lead generation and optimization steps, and the drug development process, with the pre-clinical animal models for drug efficacy and PK/PD, are still in its infancy for leishmaniasis, given that more systematic approaches to develop new chemical entities for this important but neglected disease have started only relatively recently. Therefore still much about VL chemotherapy is going to be learnt on the way of development of new drugs to treat leishmaniasis, and the real measure of success will be the delivery of these future drugs to the patients in need.
Researchers are currently doing long-term studies on women and children to explore this issue further. Ok… so it has been nearly two weeks since the end of my chemotherapy treatment. Taxol treatment #12 is over. Since October 24, 2013 I have been poisoned to be kept alive 16 times. I have been poked by over 36 needles just for blood work and chemo, not to mention 4 shots to the stomach for Neulasta, many pokes during my five-day hospital stay for Influenza A, and the many needles used on me during my surgeries. I have had 7 Tissue Expander fills for a total of 14 needles being shoved into my “Breasts”. If I never see another needle again I would be happy. Thank God I do not have a fear of needles, I guess people with Cancer or health problems that require so many must get over that fear very quickly, or take Xanex before every appointment. By the end of my Taxol treatments I had a permanent bruise from blood draws since I am now only able to use my left arm for that or for blood pressure due to the lymph nodes being removed from my other arm.

The last 10-15 years have seen a gradual but important change in the perceived role of chemotherapy in the treatment of advanced cancers. During the 1970s and much of the 1980s, the majority of oncological trial protocols concentrated on evaluating response rates, disease-free intervals, and overall survival as endpoints. Although those involved in the treatment of advanced malignancy with intravenous chemotherapy frequently observed symptom improvement in their patients, the concept of “palliative chemotherapy” was almost a contradiction in terms. Even in advanced breast cancer (where effective palliation with chemotherapy has been possible for decades), this aspect of care is still frequently undervalued in papers which concentrate on highlighting modest effects on survival.
